Time:            7-8:30 p.m.

Cost:           $10 or FREE if you pre-register 
                   before Feb. 24 by e-mail. 





Age Definitions: 2007-2008 Season

14 and Under:
Born on or after September 1, 1993
13 and Under:
Born on or after September 1, 1994
12 and Under:
Born on or after September 1, 1995

The following forms must be filled out and turned in at tryouts.


USAV Tryout Waiver Form: Please fill out the form and sign/date the bottom.

USAV Membership Form: Please be sure to fill out the top section, including birthdate. Read the back of the form and sign/date the bottom. Both the player and parent must sign the back of the form.

USAV Medical Release: Please fill out both sides of the form. There is no need to attach any immunization or other medical forms.
